Fistula Surgery in Gurgaon

An anal fistula is considered as an unusual channel through which the skin surrounding the anus communicates with its inner lining. This usually happens from an ongoing abscess of the anus that was not treated, resulting in pain, swelling and the expulsion of pus. Due to slower healing processes Anal fistulas remain quite common in India. Surgical intervention is usually the best and only way to effectively treat the disease.

How does Anal Fistula occur?

Fistulas mainly occur due to:

Infection: Fistulas are caused as a result of untreated anal abscess.

Crohn’s Disease: This case of inflammatory bowel disease enhances the possiblity of forming a fistula .

Direct Injury or Surgery: Physical trauma or surgical procedures done previously in the anal region may lead to developing fistulas as a consequence.

These are some of the common signs:

  • Pain and swelling around the anus.
  • Pus or blood discharge.
  • Skin irritation around the region.
  • The patient’s ability to excrete stool.

Assessment of Anal Fistulas

A precise diagnosis of the disease is key to administering the correct treatment. Considered procedures include:

Physical Examination: Look through the anal area for external openings or signs of infection.

Imaging Studies:

  • MRI: This procedure captures images of the region where the fistula is located and the adjacent body tissues.
  • Ultrasound: This technique is ideal for viewing the fistula and abscess.
  • Anoscopy: The procedure of visually checking the structures within the anus using a tiny scope.

Options for Fistula Surgery

Surgical treatment of anal fistulas can include:

  • Fistulotomy: The specific fistula tract’s opening is permitted to flat scar, and is opened. Although it does work well, this treatment option is not suitable for those fistulas that include considerable amount of the sphincter muscle due to possibility of incontinence.
  • Seton Placement: A surgical-grade cord, called a seton, is sutured externally over the fistula tract. This allows for the continual drainage and gradual healing of the fistula while reducing the risk of incontinence. The seton may be periodically tightened to help cut the rope which enables scarring to take place behind the fistula so the lasso can withdraw the fistula without significant amount of surgery. This technique is known to have developed from an ancient Indian surgical practice referred to as Kshar-Sutra technique.
  • Advancement Flap Procedure: Inserting the internal opening of the fistula into a flap of tissue from the rectum which is undamaged. This method is easy to complex fistulas as well as fistulas that are sclerosing the sphincter muscle.
  • LIFT Procedure (Ligation of Intersphincteric Fistula Tract): Operation can be performed by gaining entry from the fistula tract between the sphniceter muscles. This is repositioned, fused and severed below the line of ligature. The benefit known is that the cut muscle is functional and preserved, and this produced remarkable results.
  • Fibrin Glue or Collagen Plug: These techniques require the least amount of assistance as the fistula tract is sealed off by either a plug or medical-grade adhesive. These methods require less assistance but their success rate may not be as high as surgically invasive methods.

When Surgery Is an Option?

Surgical intervention is warranted under these identified circumstances:

Symptoms Persisting: Ongoing pain, discharge, and infection exhibiting no changes with the use of conservative treatment methods.

Abscesses Recurring: Repeated formation of Abscess suggestive of an underlying fistula.

Fistulas That Are Not Simple: Fistulas that consist of a number of tracts or those that comprise large portions of the sphincter muscle.

Quality of Life Impact: Performing everyday activities or cleaning oneself becomes a challenge due to the fistula.

Postoperative Care and Surgical Recovery

Recovery ranges depending on the specific technique used to surgically intervene as well as the patient. General follow-up care includes:

Pain Relief: Ordering analgesics to relieve discomfort.

Wound Management: Guidelines for keeping the operative site free from moist and dirty substances. Healing may involve Sitz baths for promotion of recovery.

Diet Change: Diet that contains a high quantity of fibers to facilitate loosening of stools and minimizing the need for straining while passing stool.

Follow Up Care: Regularly scheduled medical visits to evaluate healing and address complications should they arise.
